Title: Support for the scaling up of the national triple elimination plan and the validation process for HIV/Syphilis MTCT

🌍 Background and Rationale

Since 2021, the World Health Organization (WHO) has recommended the triple elimination of mother-to-child infections : HIV, syphilis, and hepatitis B (HBV). This strategy aims to significantly reduce maternal and infant morbidity and mortality.

In Benin, in 2023 :

  • Approximately 68,000 people are living with HIV, 65% of whom are women ;
  • HIV among pregnant women : 0.85% ;
  • Syphilis : 0.13% ;
  • Hepatitis B (HBV) : 6.58%.

⚖️ Pilot phase : actions and results (2022-2023)

As part of the Global Fund’s NFM3 grant, Benin conducted a pilot phase in two health zones : Savè-Ouéssè (SAO), Comé-Bopa-Grand-Popo-Hilacondji (CBGH). This phase targeted 10% of pregnant women with an integrated service offering. It achieved very satisfactory results between 2022 and 2023. Of the 32,044 pregnant women seen in ANC in the pilot areas, 99.9% were screened for HIV, 99.5% for syphilis and 75% for HBV. These efforts led to the identification of :

  • 122 cases of VIH ;
  • 28 cases of syphilis ;
  • 335 cases of hepatitis B.

  • Insufficient documentation of HIV/HBV coinfections
  • Logistical difficulties in transporting samples (HBV viral load)
  • Limited stocks of HBV reagents
  • Incomplete monitoring of the mother-child pair
  • Low screening of partners (19%)

With the support of GC7, the PSLS plans a gradual expansion of triple elimination across the entire country, with a view to obtaining WHO certification. A contextually appropriate and tailored strategy is essential to ensure the sustainability and impact of the interventions.

To better prepare for the scaling up of triple elimination in Benin, the PSLS intends to conduct an in-depth evaluation of this pilot project, which will be carried out in 2022-2023, to provide a more detailed analysis of the challenges encountered and identify adjustments to be made. This is the reason for this request for technical support, supported by Initiative/Expertise France.

Main objective : Support the scaling up of the triple elimination strategy for HIV, syphilis and hepatitis B, and support the validation process of HIV/Syphilis ETME in Benin.

  • R1 : Initial evaluation of the pilot implementation
  • R2 : National triple elimination strategy developed and budgeted
  • R3 : Revised training, monitoring and evaluation tools
  • R4 : Support for the ETME HIV/Syphilis validation process
